Concurrent request Log and Output does not open in a browser

Dear Oracle Gurus!

The concurrent request log and output does not open up in the browser directly and we are having to go to 'Tools' menu and select 'Copy File' to be able to view the output on the browser. Have anyone else faced this and how can this be fixed?
