Upgrade 12.1 to 12.2

Hi;

Currently we are using Oracle EBS Version 12.1.2 modules Oracle Financial, Supply Chain (INV, PUR, OM) and Payroll and now we want to upgrade release to 12.2. Please advise how to proceed on this to plan it properly. Please share your experience and relevant documents to start work on it.
