RUP 20000288 patch fails to load: A fatal error related to codelines or codelevels has occurred

I get the following errors when apply RUP8 patch - p20000288_R12.HR_PF.B_R12_LINUX

Successfully read patch driver file.

Determining target release...

Current target    release is  12.1.3

Warning:

The description for the entity 'pay' in the patch driver
does not match with the current description.

The current description is      :'Payroll'
The description in the patch is :'Oracle Payroll'

The current description will not be changed.

AutoPatch error:
--------
A fatal error related to codelines or codelevels has occurred.

See messages above for details.

Does anyone know how to solve this?
