ACA Source for MEC?

We have one plan that is lumping everyone into the 1H/2C category.  The ACA details are set up identically to our other plans, but for some reason Line 14 won't flag this as MEC=Y, even though Line 16 determines it is in fact MEC.  Any ideas on where we can look on this plan specifically that would cause the 1H?
