Troubleshooting Tips Needed on Oracle Alert on QTY ORD Hold

We have an Alert setup on the Qty Ord Holds which seem to be working fine for awhile now.  But our users noticed that there was a Hold on there Hold Report but an email was not received.  When I look in the History for Checks on the Alert it shows it did find an exception and there were four actions.  Seems right.  But then I look at Actions and for the same date and time, the action is empty.  How can I tell what happened and why it didn't send the four emails?  When I run the sql manually
