Project is capitalized twice - Asset was already capitalized before

In project #192 task 07.01 we received a total cost of $39K back in September 2015. We split this cost even in two assets and transferred to Fixed Assets (198458 and 198459). On November this same $39K were showing up as "Pending" to transfer to FA so we interfaced this cost again and it went thru as a cost adjustment to the asset 198458. Now we have the problem that the total cost of the asset is incorrect and the CIP has been credited twice for the same amount.

Could you please take a look and let me know how could this happen? I need to understand if it is a system issue or a process issue.
