ACA Reporting in EBS - Part III on 1095-C is unexpectedly blank for Self Insured employer

We are a self-insured employer using EBS and Standard Benefits. The plans are all marked Self Insured in the Primary Funding Method, date-tracked back to Jan. 1, 2015. However, Part III of the 1095-C forms is blank and the "covered individuals" box in that section is not checked.  Hoping we just missed a simple setup step. Please advise.
