install error, logp.251 - Error code : 101

So, I fired off my install of 12.1.0.5 on one host pointing to the repository on another host.  Came back today and it failed.  I wonder if you can help figure out why.

Console reports: Configuration assistant "OMS Configuration" has failed

cfgfw/oracle.sysman.top.oms_2016...log:

INFO: oracle.sysman.top.oms:Repository Creation Utility - Create : Operation Completed

INFO: oracle.sysman.top.oms:OMSCA-ERR:Post deploy operations failed. Check the trace file:/apps/database/dbs/db01/middleware/oms/cfgtoollogs/omsca/omsca_20160114174143.log
