Users Detailed Responsibilities

Dear All,

Our Audit firm asking urgently for detailed users responsibilities (Modules, web windows, forms and reports under each responsibility, and what permission they are granted (Insert, Update, delete) for each web window and form...)

Is there any report or SQL statement satisfies their need...

We are using ORACLE EBS R12.1.2
