How to Read and Write a CSV file

Dear All

My requirement is to read and write to a csv file and insert data into table. I am working in EBS environment having version 12.1.3 and database version 11.1.0.7.0 I want know about following.

1. Where should I place/create my csv file?

2. How should I create directory?

3. What procedure should I write to get data from csv file? (I have written a procedure but it is throwing error of Invalid Operation.)
