Unable to produce COBRA letters

We are attempting to set up COBRA administration and have gone through the setup documentation. We have added Qualifying events and set up all constants.

We are testing by Terminating an employee and attempting to generate the COBRA Election Notice from the Work With Qualifying Events form. The Qualifying Event does show in the grid, however clicking on Produce Letters does nothing. Nothing shows up under Form>Display Letters. No errors display. Nothing in the Application Server or Enterprise Server logs.

The docs I've found imply that the Qualifying Event needs to be manually entered, but ours is being entered automatically when we terminate the employee. Is there some step we are missing?
