CDL not created

This week I discovered two lines in a Pre-Approved batch entered last September where CDL's were not created until January.  The two transactions offset each other so we did not catch the problem in our balancing routines.  We check the exception reports before closing the PA and PA/GL periods each month and there were no exceptions in September.  Has anyone encountered this problem before?  I could not find a related topic in Metalink.
