OAB Issue with FSA/HSA Open Enrollment Deductions

During Open Enrollment. those employees who wanted to keep their same coverage and did not 'touch' their benefits did not get their deductions taken for FSA and HSA plans beginning in 2016.  The rate is missing for these plans only.  Their other benefit plans did receive rates and the element entries were updated.

Did anybody else have this issue and what was the solution?  I think the element entries for FSA/HSA were end dated and not restarted.
