FSC is applied outside the accessorial effective and expiration dates

We are having an issue with shipments being created and FSC applied even though the shipment is outside the FSC accessorial effective and expiration dates.

FSC accessorial has effective date of 2/16 and expiration date of 2/22.

Shipment is created on 2/19 for a start time of 2/29. The shipment is created with the FSC applied.

If the shipment is recalculated, the FSC is promptly removed.

This is OTM 6.4.1.

Any ideas?
