Material Workbench Form Displays Duplicate Locations

We use Project and Task and we are seeing same location displayed twice for the same item in the Material Workbench Form.

For example - Material workbench Displays -

LocatorName.Project.Task          PART#          1

LocatorName.Project.Task          PART#          1


I checked MOQD, PART# is only in quantity 1 in this locator. Also even if the quantity were 2, it should have shown

LocatorName.Project.Task          PART#          2


If you have seen this before, please let me know what you did to resolve it.
