R12.1.3: Standard Manager - Custom Program - ends with status of ERROR even though there are none

We have a custom program that is completing with a status of Error even though there are no errors and completes with 0

It is working fine on Production but during regression testing of RPC4 and January CPU 2016 patches we found this issue.  Dev team has analyzed code, debugged etc, and we can not figure out the cause. 

Ideas?
