orachk reports wrong CRS Home - Version

Hi all,
orachk reports wrong CRS home version (CRS Home - Version /u01/oracle/11.2.0/grid_4 - 11.2.0.2.0) in orachk pre upgrade readiness report where as the CRS home installed is 11.2.0.4 . Report advises patches which are of lower patch level than current CRS home as a prerequisite for 12.1.0.2 upgrade.
Please find output of OraChk.
