11i Code combinations are autocreated for a disabled segment

In  11i    Code combinations are auto created for a disabled segment when there is a transaction.

User did a OM Shipments for which code combinations were not available and one of the segment was disabled, but when   journals posted to GL , system auto created the code combination with a description as 'Record Auto Added For Gemms Doc Type OMSO'.

Please help to understand the functionality here as one of the segment value is disabled but still it created normally.
