r12.2 New Ledger setup - System-generated Data Access Set not visible

Greetings -

We are in the midst of crating a new ledger in  r12.2.  While under Accounting Setup Manager> Primary Ledger> Operating Units - Update section, we are unable to add operating unit.  Message received is  java.sql.SQL.Exception: ORA-20001: An organization with this name already exists.  Please enter another name for the organization that you want to create.  ORA-06512 at "APPS.HR_ORGANIZATION_API", line 6093 ORA-06512 at line 1.  Yet, if we just try to query for existing ones, it doesn't return any values.

We found a note which mentioned going thru the Open Organization Form.  When we try set it up there, we get a no LOV error when trying to populate Default Legal Context. Since Default Legal Context is a required field, we are unable to complete our Accounting Setup steps.
