USD Override showed up from no where, how to correct

Using HFM 11.1.2.2.500

I have a base entity member that does not have an override entered under <entity currency> nor does it have anything in <entity curr adj>, but there is a value in the <entity curr total>.  There should not be any override in these intersections.

Main question is how do I correct that?  I've tried CAWD, but no change.

Secondary question, where would something like this come from?
