Create Custom Print with Adobe LifeCycle

I am trying to create a custom print for an Email Notification in a workflow for my Director using Adobe LifeCyle Designer version 8.2.  I am able to create the form and everything works except for displaying General Comments properly.  I need the form to be dynamic and show all of the General Comments.  I have tried multiple changes in settings, use of subforms, but I never get multiple General Comments to show up.  Does anyone know what the trick is to make it work?  Thanks.
