Cloned WF BENAPPRV not working - Failed to Initiate ESS Workflow Approval

Hello,

We are at tools 9.1.5

Out of box, HR has a WF BENAPPRV and we want to tweak it a little bit.

So I cloned the WFBENAPPRV  to BENAPPRV01 and didn't do any change yet.

From P085515, I changed the event CHD(Children) to make it call WF BENAPPRV01.

The new WF is validated from Workflow modeler and it went through DV build and is activated as well.

When I triggered it from P05410(Self Service Director), after "I accept" the button is clicked(P085564), I got an email

Email subject:  Failed to Initiate ESS Workflow Approval

Email body is like this:
