Attachment exists but not visible to user

An attachment exists on the RMA, however, unless the user clicks on the attachment to review they do not know it exists. The paperclip does not display.

I have the same result on both P40051 and P400511. Could not find a bug noted during a search.

I'm on E1 9.1 tools release 9.1.5
