Print information only on first page

I'm trying to print a payment stub on an invoice on the first page and not on any succeeding pages. I know I can use the header / footer feature from Word and use a different header / footer on the first page but the payment stub requires me to use some form fields which are not usable when I put them on the footer.

Is there a way to accomplish this without using footers?

I have attached a sample template and the XML data.
