Seed_Database.ctl file is missing

Seed_Database.ctl file is missing during dbca database creation.  Here is the content of the log:

Unique database identifier check passed.

/home/ has enough space. Required space is 4560 MB , available space is 33918 MB.

/ has enough space. Required space is 1580 MB , available space is 38864 MB.

File Validations Successful.

Cleaning up failed steps

DBCA_PROGRESS : 5%

Copying database files

DBCA_PROGRESS : 7%

File not found: /home/oracle/app/oracle/product/12.1.0/dbhome_1/assistants/dbca/templates/Seed_Database.ctl

DBCA_PROGRESS : DBCA Operation failed.

Sure enough, the file doesn't exist.  There is a cdb file, but not a ctl file.  I see the file in my 12.1.0.1.0 system, but not
