How to permanently fix "fault.io.pci.device-invreq"

Hi,

I have seen this problem in T4 many times till i lost count of the number of times.

We can perform fmadm repair to clear this fault. This fault doesn't seem to affect anything but it is annoying as it comes back after some time or after performing a reboot.

Is there a permanent way to fix this problem? i remembered 1 of the Oracle engineer had recommended to apply a patch but i cannot remember which patch it is?

Is this an actual fault? I have seen this too many times and it seems to be a bug instead of a fault.
