Documentation for Standard 12.1.0.1 DB upgrade to Standard 2 12.1.0.2 DB

I am preparing to upgrade a 12.1.0.1 Standard Database to 12.1.0.2 (Standard 2).  I am looking for some specific documentation for the upgrade.  Where are the Grid Installation files?  They are not included with V77388-01 on the Delivery site.  Are there any documents specifically discussing Standard 2?

The underlying server is RHEL 7.2

Any feedback is much appreciated.
