JDE E1 9.0 ACA program P08121 form W08121 C 1094C part IV import error

In JDE E1 9.0 ACA program P08121, has anyone tried to import 1094C other ALE members with csv file?  There are only two fields in record, Name and EIN, and the name field is grayed out and suppressed during import.  Then after a save and exit of program, the other ALE members records are not saved.  This issue occurred both before and after ESU JL23683 was installed.  We have some EIN members who are not in our JDE ERP, so we have to either import or manually enter data into part IV.  I cannot navigate to part IV to type
