what patches do I need to apply - RAC DB

Oracle database = 11.2.0.4.8

OS = Linux 6

Number of nodes = 4 (RAC DB)

Storage = ASM

I need to figure out the following couple of questions:

1. How would I find the latest 12c patch set(current 11.2.0.4.8 to 12c) I can apply to my cluster/ASM RAC database ?

2. How to figure out what all patches do I need to apply on the quarterly basis? ( any print screen example and/or document link would be appreciated.)
