ODA V1 System Disks are now getting old: you have a much higher chance of detecting your SYSTEM/BOOT

IF you are using V1 you have a much higher chance of seeing your SYSTEM/BOOT disk fail IF it was in a predictive failure state after the required power cycle for patching.

While I have seen a few SRs where users think the problem is due to the patch / upgrade, I would tend to agree with our HW group's assessment that the problem is more likely to be detected as a result of the Patching process.

The age of the V1 now justifies special attention for system disks, but consider running the following on both of your V1 ODA servers.
