Why are column fields disabled on FMLA substitution rules?

I'm setting up Global Leave for FMLA.  When I try to add substitution rules (P07603), all column fields are disabled except for the leave type field.  It seems I'm missing something in my set up.  I have reviewed the following several times:

  • Leave Company set up at default level (0 Company) - Medical tracking and substitution checked.
  • Processing options validated
  • Pay Types set to track leave
  • Leave Type set up and active, including FO, FS, FV
  • Leave Group created and built
  • Leave Sets and Verification rules are set up
