DFF segment field update tracking

Hello,

Want to know is there a way to track changes made on a dff segment field and display it in the form/page with  who updated last with date and  time which will be visible for any user requerying the form/page.

Attached screenshot with details on what we are looking for.

Any input on this is really helpful.
