Saved forms and data models disappearing from BI Publisher

We have 2 different users that are having issues with items disappearing from BI Publisher. 

user 1 created a custom version of a standard report (added a logo or signature text) using the Word Plugin and did a "upload template as" back to BIP to the original location of the form (F_CO_03). It was assigned as the default and imported to Contract Management 14.2. It was working fine last week. She went in to do additional testing this week and the form is gone. We have checked the repository folders on the server and it is not there anywhere. We have looked on the local drive to ensure it was not saved there.
