Grouping in custom Dunning Letter layout in R12

We are upgrading to R12 and in a process of a customization of the pre-defined standard Oracle Dunning Letter (EBS). I need to display transactions by group Debit/Credit, just like 11i Dunning Letter currently shows the data.

1) How do I add a group (debit/credit) above the payment_history detail cursor/loop that is defined  in the standard pre-defined query? It is based on a query defined in EBS Advanced Collections

2) Do I need to install BI Publisher and open the .rtf file in it or it is enough to open the existing Oracle .rtf layout for the Dunning Letter  and customize?
