credit note approval workflow do not flow to correct approver

Hi All

we have in place for now one month credit note approval workflow

with standard feature

and notification should flow to profile option setup  : OM: Notification Approver

it works for most of our RMA but with some of them notification goes to another employee

thanks for your help to understnad why some notification do not flow to correct approver
