oracle 12c crs install failed on ssecond node

Linux 6.5 x86 64, oracle 12 c two nodes.

runcluvfy.sh passed except Swap (required 16g, I have 13.xxgb) and warning on /tmp (

WARNING:

The size of in-memory file system mounted at /dev/shm is "32768" megabytes which does not match the size in /etc/fstab as "0" megabytes)

installation from first node get error:

Error while copying directory /opt/app/12.1.0/grid with exclude file list '/opt/temp/OraInstall2016-06-01_02-46-21PM/installExcludeFile.lst' to
nodes 'sso-ata50a'.
