Record not loaded in FDMEE

Has anyone experienced a missed record when loading to FDMEE?  Every once in a while, our file load to FDMEE would miss one record.  All other records of the same entity would be loaded fine.  Similarly, all other records of the same account, or same ICP, or same custom dimension, etc would all be loaded fine.  There's nothing special and it would be only one record that would be missed.  Then we would reload, and all records would be loaded this time.
