Authoritative 1094C Minimum Essential Coverage Offer Indicator

The 1094-C Transmittal is showing "N" in the "All 12 Months" column in Part III ALE Member Information Monthly section for us. Has anyone else encountered this?  We provide "minimum essential coverage to at least 95% of its full-time employees and their dependents for the entire calendar year" as per the IRS instructions, so we're expecting a "Y" in this column. I confirmed that we have the "Provides Minimum Essential Coverage" and the "Provides Minimum Value" fields set to "Yes" on the Extra Plan Info screen for our medical plan. What generates the setting for this column?
