AX Payables Reconcilied Unaccounted

Hi

We have recently started to use Cash Mangement and we ran into an issue when Running the Post Manager for AX Pyables. Error below. We have payments in Oracle with status reconciled unaccounted, and accoutned status on payments of Partial

Can anyone confirm if there is a set up issue we missed?

425 events generated a recoverable failure.                                                                                   
   1517432 CASH_CLEARED                  4399356 An unexpected failure occurred during the event translation. Internal   
location failure: ax_tp_data_pkgInsertHeader                            
Please contact your system administrator.                               
Additional information :                                                
Send_message returned ORA-00001: unique constraint                      
(AX.AX_SLE_HEADERS_PK) violated
