cannot change the quality disposition of an item sample.

There is an issue in an item sample.When trying to change the disposition to Accept from complete, the system gets freeze.(Oracle forms application should be re-start to continue)

When re querying the particular sample  an error message pops as "FRM-40501: ORACLE error: unable to reserve record for update or delete".

After some tries sample will be displayed but with the disposition as complete.

Same issue keeps on happening

What could be the reasons?
