ACA 1094 AIRBR1011 Error for a few employees

We are getting an AIRBR1011 (Employee's Share of Lowest Cost Monthly Premium (annual and monthly) is missing) on the return file from the IRS on about 1% of our employees.  Some of these employees are 1A across the board, some changed status during the year, we can't find a discernible pattern.  Has anyone else run across this?
