ACA report pulling in dependents

I am on 9.0 I have the recent ESU JL23825 and I have finished my test 7-0 but now I am testing my data and I am pulling in the dependents.  I know I saw that question before but there are so many posts out here I could not find the answer.  Also I am adding companies to part IV and I am not seeing them anywhere in the file.  I know I am running  out of time.  Help!!!
