TPE 1204 Error on production ACA file

Using Oracle Advanced Benefits (EBS).

When submitting the our production file to the IRS we get the TPE1204 error.  I see that this is in other formats as well but nothing logged for EBS that I can see.   Please advise on a fix for this ASAP.

What characters need to be removed from this XML file?
