Why would phase 2 Pick Confirm not update the Level 1 Qty and UOM in the F4611

A few occurrences of the F4611 Level 1 Qty and Level1 UOM for Phase 2 Pick Confirm are not updated. Therefore stopping the item from being processed. Why would this be occurring?
