Cannot ship confirm internal sales order

Our client has an internal sales order that they are trying to progress.  The internal sales order was created in August 2015, and one line was cancelled via the requisition.  The remaining order lines had a "hold" placed against them when the one line was cancelled, and the holds weren't released.  The inventory stock has been moved to the staging subinventory but never released, and stock is now reserved for this order.  We obtained a datafix to remove the holds from the remaining lines, but cannot ship confirm the order, as we now get an error : "APP-ONT-251015".  When checking the workflow, this shows as being active for the lines that have had the holds removed.  I have tried to suspend and resume the workflows but this has not resolved the issue.  Is there any way to progress this Sales Order??  We cannot cancel the order lines as we get an error : "you are not allowed to cancel Order Line because: Line has been pick confirmed/staged".
