ACA Manifest file failed due to xml format RD08151

XML manifest file keeps rejecting.  THere was data missing from the manifest file,  I changed it manually in notepad, and now the message received deals with the format.  Please latest attached file.  I need to correct the format.  Thanks in advance for help!  We also have an  SR 3-12954839311 : manifest file failed - need correct format.
