ACA Reporting - Manually Entered 1094C not saving

JD Edwards Enterprise One Version 9.0

When manually entering 1094c using P08121.  Using Add button, then completing form and clicking OK to save.  Form clears as if record has been saved, but upon returning to P08121 and clicking find, no record has been created.  Received no errors or warnings.  Security is not an issue.

What is preventing the P08121 from saving the record being manually entered?
