upgrade to 12.1.2.6.0 - read failed after 0 of 4096 at 0: Input/output error

When running the precheck to upgrade ODA Bare Metal V1 from 12.1.2.5.0 to 12.1.2.6.0, I get the following input/output errors in node-02. If I ran the same on node-01, I don't see the errors.
