I have unposted headers in F0413 and F0414 but posted in both F0011 and F0911

I have 3 sets of batches as described above showing on integrities R007021 and R04702A.  According to Oracle doc flowcharts, I do not have an integrity issue; however, warnings that direct database fixes should only be done as last resort.  We do not know how these errors were created (how does a user post a header without posting the detail?).  How are these errors created and how do we fix this without directly in the database by SQL the F0413 and F0414 tables?
