One PO does not have any header and line info printed

We are using EBS version 12.2.4 with database version 11.2.0.4

We have one PO when run the po output for communication request was able to generate PDF, but the header and line info did not get printed.

All other PO was generated fine. Only this particular PO is having issue.

In the test instance that was cloned the past Sunday, this PO was still able to generate PDF with header and line info printed on it.

Not sure what user did to modify the header and now in production, the header and line info does not get printed anymore.

How can we go about investigate and trouble shoot this? Gurus, please advise. Thank you.
